The Cougar River Subdivision is a four-level HO scale layout in Manitoba, Canada. It is owned by my brother-in-law; it's been my good fortune to help him a bit with the layout, and to be able to run on it.
The layout--one of the largest in Winnipeg--features both CN and the CPR. It does not focus on a particular region. The trains runs along the walls from room-to-room throughout the whole basement (furnace area, past the bathroom, back room, spare room, rec room); a helix in the pantry is used to connect the levels. The upper staging yard is suspended from the ceiling. Altogether, it shows that you can have large layout, and room for the family, too.
Unique for a model railroad, it has a scale-size baseball diamond.

A view of the lower level (and helix) can be found at http://www.youtube.com/watch?v=bqmE6bpf3_E
